Critical Connections Advance Health Equity
Founded in 1971, Family Tree Clinic is a regional leader in community-based and equity-focused sexual health care and education. They reach over 22,000 people a year through their medical clinic and education services. In the Fall of 2021, Family Tree opened its new and expanded health care facility at 1919 Nicollet Avenue, across from Plymouth. This trauma-informed, art-infused, and community-centered health facility is deeply rooted in the organizational commitment to health equity. They are thrilled their new facility is being built in the neighborhood with their highest patient growth and couldn’t be more pleased to bring excellent, inclusive care into the centrally-located community of Stevens Square with the tremendous partnership opportunities with Plymouth, Groveland Food Shelf, Old School by Steeple People, African American AIDS Task Force, Stevens Square Community Organization and many more. Family Tree believes that authentic relationships are at the heart of the solutions to some of our most chronic challenges, so they look forward to deepening their many connections to the Plymouth community.
